JPET Policies and Procedures
Plan Changes:
1. New Enrollments: Please email or fax a completed employee enrollment application and worksheet to apeyton@azpcc.org. Confirmation will be emailed within 24-48 hours of enrollment. All enrollments submitted after the 15th of the month will be reflected on the next months invoice. Qualifying events for new enrollments include:
a. New Hire
b. Fulfillment of Waiting Period
c. Status Change (Part Time to Full Time)
d. Return from Leave
e. Loss of Prior Coverage
2. Coverage Changes:
a. Demographic Information: please email or fax requested change to apeyton@azpcc.org
b. Product Information: plan changes can only take place upon renewal; any desired changes will need to be wait until August 1, 2009.
c. Dependent Coverage: dependent coverage changes can only take place upon renewal; any desired changes will need to wait until August 1, 2009 unless there is a qualifying event. Qualifying events include:
i. Loss of prior coverage
ii. Birth
iii. Marriage
iv. Adoption
3. Terminations: Please send an email to apeyton@azpcc.org with the following information in the body:
a. School Name
b. Individual’s Name
c. Termination Effective Date
d. Reason for Termination of Coverage:
i. Termination of Employment
ii. Divorce
iii. Moved
iv. Dependent reached maximum age
v. Death
Confirmation of termination will be emailed within 24-48hours and COBRA services will be copied on the email. All terminations submitted after the 15th of the month will be reflected on the next month's invoice.
4. COBRA Policies:
a. COBRA will only be offered to individuals of groups that are currently enrolled and active in JPET
b. JPET provides notification for COBRA processing to United Healthcare provided services ONLY. This includes medical, dental, life and critical illness policies. All other benefits contracted through other carriers (Sightcare, Transamerica, AdminPro) are subject to notification by the agent of record or school and not JPET.
c. COBRA will be copied on all confirmation of termination emails to serve as notification to begin processing. JPET will provide all necessary information in the body and attachments of this email.
d. Processing can take up to 14 business days – please allow for this time to lapse before contacting JPET or COBRA for inquiries on specific enrollees
e. Please allow another 3-5 business days for mailed delivery of enrollment packets
f. All payments from the COBRA enrollee to UnitedHealthcare should go directly to JPET. If for some reason the school should get the check, it should be signed over and forwarded immediately to JPET.
